The referral process for neurodevelopmental assessments includes assessments for:
- Autism Spectrum Disorders (ASD)
- Attention Deficit Hyperactivity Disorder (ADHD)
- Tics
- Learning, sensory and coordination difficulties
- Other neurodevelopmental conditions
Please note, there are three parts to this referral and all sections need to be completed and returned including:
Form A | Online referral form
This needs to be completed by a professional who knows the child / young person in a professional capacity. eg school, GP, other health or social care professional. Please note this form can be completed by your child’s school along with Form C.
Form B | Information from the parent
Please note that this will be sent to you after Form A has been received from a professional working with your child / young person. You will receive a link to an online form to complete.
Form C | Information from the school or another professional (if electively home educated)
NB the professional who completing Form A will need to provide an email address for the young person's parent / carer and school. Links for Forms B and C will then be sent to these email addresses for completion.
Once all electronic forms have been received, they will be screened to establish the best course of action for the child’s needs and additional information gathered to inform this decision.
Please note: if you feel this young person needs to be assessed for ADHD, there is an expectation that a parenting programme is completed in the first instance.
NB: Children under the age of 5½ where ASD is suspected will be passed to the Community Paediatricians and Speech and Language Therapists. This will be in the first instance if they are not already open to these services.
